Your first appointment with our practice is an important educational visit for both the patient and our team. During this appointment, we will discuss your current concerns and provide information regarding the recommended treatment options. The surgical and anesthetic procedures involved in your care will be explained thoroughly to help you feel informed, comfortable, and confident about your treatment.

Many procedures can be completed during the initial visit. For more extensive treatment, additional appointments may be required.

IMPORTANT

For any patient under the age of 18, a parent or legal guardian must be present during the initial consultation and on the day of treatment.

Please bring the following items with you to your appointment:

REFERRAL SLIP:
A referral slip indicating the reason for your visit.

MEDICATIONS:
A list of all medications you are currently taking, including dosages if possible.

MEDICAL CONDITIONS:
Information regarding any current medical conditions or health concerns. If you are unsure whether a condition may be relevant to your treatment, please discuss it with our office during your consultation.

SURGICAL HISTORY:
Information regarding any previous surgeries or hospitalizations.

INSURANCE INFORMATION:
Please bring all applicable medical and dental insurance information. Providing this information will allow us to assist you in obtaining any available insurance benefits or reimbursement.

X-RAYS:
If you have any x-rays or imaging relevant to your treatment, please bring them with you to your appointment.
